Chloe Valentine inquest
The Chloe Valentine inquest progress report sets out departmental and government actions in response to each of the recommendations handed down in the findings of the coronial inquest into the death of Chloe Valentine.
In 2004, the Parliament of South Australia enacted legislation to establish a commission of inquiry into children in state care. A separate inquiry was later established into children on the Aṉangu Pitjantjatjara Yankunytjatjara (APY) Lands.
Commissioner Ted Mullighan presented the reports of the commissions of inquiry to the South Australian Government on 31 March 2008 and 30 April 2008.

In 2002 the Honourable Robyn Layton QC was commissioned to review South Australia’s child protection system.
The Child Protection Review report, Our best investment: A state plan to protect and advance the interests of children, (PDF, 1.9 MB) was submitted to the South Australian Government in March 2003.
It contained 206 recommendations for reform across all agencies to improve the child protection system and make our state safer for our children.
The South Australian Government’s response to these recommendations included:
- establishing the Child Death and Serious Injury Review Committee
- establishing the Office of the Guardian for Children and Young People
- expanding the range of people required by law to report suspected child abuse and neglect to include, among others, ministers of religion, child care workers and sporting groups.
