Transitioning young people from leaving care to an independent life requires guidance and tailored involvement of a range of agencies to prepare and plan for a young person's exit from the child protection system.

Reforms will establish mechanisms across government and non-government agencies to deliver housing, health care, further education, financial and employment support, to give young people coming out of care the best chance of a settled and fulfilling life.
